  1. Application must be completed by the applicant.  Please type or print clearly. 
  1. All applicants must include a typed narrative of 400 – 750 words responding to the following prompt:

Describe your most meaningful achievements and how they relate to your intended field of study. 

  1. Additional documents required for completed application:

High School Senior Greater Columbia Area Planning to Attend a College or University And

High School Senior Greater Columbia Area Planning to Attend a Historically Black College or University (HBCU)

 

·        Three (3) letters of recommendation from the following:

1. A current school guidance counselor

2. A teacher, current or past.

3. A character reference letter - One of the persons listed as a reference on the application may be used.  The person writing the letter should be someone other than a relative, who knows the applicant well, (i.e., a minister, neighbor or supervisor).

·        Copy of College Acceptance Letter

·        Official Transcript   **Minimum GPA 3.0 on 4.0 scale

·        All letters must be signed and original

 

Artistically Talented High School Senior Greater Columbia Area Planning to Major in the Arts – Visual Arts, Music, Dance, Creative Writing or Drama

·        Three (3) letters of recommendation from the following:

1.      A current school guidance counselor

2.      A creative/performing arts teacher, coach or advisor

3.      A character reference letter - One of the persons listed as a reference on the application may be used.  The person writing the letter should be someone other than a relative, who knows the applicant well, (i.e., a minister, neighbor or supervisor).

·        Art portfolio, programs of recitals, public performances or other evidence of outstanding achievements

·        Copy of College Acceptance Letter

·        Official transcript

·        All letters must be signed and original
